Course Description

Explore the concepts of health in Canada, with a focus on the impact of health transitions on various stakeholders in different circumstances and contexts. Transitions in health occur often during the life span of individuals, families, groups, and communities. Healthcare transitions involve assessing, planning, and coordinating effective continuity of care to clients who experience a change in health status, transfer providers and locations, or require support managing ongoing health needs. Using a strength-based, health promotion lens, you will learn to support and advocate for their clients during their health and wellness journey. The passing grade for this course is 65%. 

This course has been funded through the Ontario government's Nursing Program Transformation in Ontario's Colleges initiative.
