Course Description

Understand the complexities related to the lived experience of individuals living with and families affected by kidney disease. Person-centred chronic disease management models are used to analyze the appropriateness and comprehensiveness of design and delivery aspects of systems, services and supports for individuals, communities and populations at risk for or living with kidney disease.
